We are currently recruiting for a permanent position for a Community Nurse to work as part of our Residential and Community team. This is a full-time, permanent, pensionable position for a basic 39-hour week. There is a 6 month probationary period applying to the employment.
Job Purpose & Role:
To provide support and enablement in relation to the medical needs of our Core Members, to engage in health promotion and to provide training and advice to the Staff, Assistants and Volunteers who support the Core Members
The Community Nurse collaborates with the Core Members, Community Leader, Service Manager/Person in Charge, Day Service Co-ordinator, House Leaders and Care Team.
All duties are to be carried out in line with the L'Arche ethos, An Bord Altranais guidelines and HIQA standards.
Liaise with the Doctor and allied professionals in relation to health issues
to have good protocols in place for minor illness, PRN use, long term conditions and overall medical care for residents.
Accompanying residents to GP, specialist or hospital appointments as needed and advocating for them around their health issues
Advising Staff and Care Assistants with appropriate questions to ask etc., when accompanying residents to their GP/Doctor appointments
Establishing with Staff & Care Assistants, an understanding of medical issues, and interventions conditions.
Assisting Core Members in understanding their medical issues, their medication, and their health promotion.
Assisting in Care Planning for Core Members monitoring progress in health issues
arranging of annual medical checks
Supporting Core Members who have mental health issues and referring them to the appropriate sources of expertise
Liaising with Mental Health PHN
Conduct Risk Assessments in relation to personal care and to treatments and long-term care
Keeping up-to-date medical records for each Core Member: obtaining, reviewing and filing reports (e.g. following doctor's visits), ensuring resident's medical files are up-to-date and relevant people are informed of changes in health, medication changes etc. in association with House and Day Service teams.
Liaising with families of Core Members regarding health issues
Dealing with minor wounds/illness, changing dressings for the Core Members
Health promotion: promoting health among Core Members, this will include areas such as diet, exercise, hygiene, personal grooming, sexuality and relationships.
Training in medical conditions and treatments associated with these conditions
Training in Administration of medication, care of drugs and their side effects
Assisting in the creation and updating of Policy, Protocols, and procedures in relation to health issues
Adhering to and training Staff & Assistants in the policy relating to standard precautions checking blood glucose monitoring, blood and body fluid spills, needle stick injury.
